By completing this form, I authorize Belforest Water System to terminate the water service at the address provided. I acknowledge that I am the account holder for this address and have the authority to cancel the water service. I additionally understand that any and all monies due to Belforest Water system will be deducted from the service deposit on this account.

NOTE:

• If your home or business will be vacant, we recommend the service to be terminated on the date of vacancy. If you opt to leave water service on at a vacant home, you will responsible for ALL water usage.
  
  
• Belforest Water will terminate water within 24-48 working hours of date requested. BWS will not be responsible for water usage during that time.
  
  
• If account is subject to a refund, it will be mailed to forwarding address provided within 60 days of termination date.
